Registration of Feed Establishment and Feed Products

Any person, partnership, firm, corporation, or association desiring to engage in the manufacture, importation, sale of distribution of feeds or feeding staff shall first be registered with the Bureau of Animal Industry.

Feed Retailer - refers to an establishment engaged in the business of selling feeds or feed ingredients directly to end-users in retail quantities.

Feed Dealer - refers to a feed establishment engaged in the business of selling feeds or feed ingredients in bulk (bags) carrying differrent brands of feed products, A dealer may also be called a Wholesaler.

Feed Distributor - refers to a feed establishment that is an agent or outlet of a feed or deed ingrredient manufacturer, trader, or Importer for the purpose of marketing or distributing its products.

Feed Repacker - refers to an establishment engaged in the business of repacking feeds, feed ingredients, feed supplements, feed additives, base mixes, concentrates, specialty feeds preparations and/or other feed products whether local or imported, distribution and/or sale.

GENERAL REQUIREMENTS

A. Photocopy of Business/Mayor's Permit for the current year.

B. Duly accomplish and notarized application form 1.

C. Proof of Payment of Registration Feed.

D. Latest 1" x 1" ID Picture

REGISTRATION FEE

A. Feed Retailer
     P 60.00/year
B. Feed Supplier/Dealer
     P 240.00/year
C. Feed Distributor/Repacker
     P 120.00/year

EXPIRATION

The Validity of registration shall automatically expire on the 31st of December of the year. Application for renewal of registration shall be filled on or before January 21st of every year.

PENALTY

15days after deadline = 25% surcharge
30days after deadline = 50% surcharge
45days after deadline = 75% surcharge
45days Beyond = 100% surcharge


OFFENSES
It shall be unlawful for any person, partnership, firm, cooperative, corporation or association to engage in the manufacture, importation, sale or distribution of feeds or feeding stuff, without having first registered with the Bureau of Animal Industry.

INSPECTION AND MONITORING
Deputized inspectors shall be permitted at all reasonable times to enter and inspect any premises and / or conveyances in which feeds or feed ingredients are sold, produced, processed, transported, or held in possession for sale or distribution, and take samples for analysis without cost.
